Scope of Services
The successful service provider will render holistic core employee wellness services to SBDM employees for a three-year period. The programme covers six key areas:
Health Screening:
- On-site health screening facility once per annum
- Physical medical examinations for all employees
- Blood pressure, cholesterol, and diabetes checks
- Mammograms, pap smears, and prostate cancer screening
Employee Assistance Programme (EAP):
- 24/7 toll-free professional line support
- Online support services through wellness intranet
- Psychological and psychosocial services
- Face-to-face counselling (up to six sessions per employee per case)
- Financial, legal, health, and life management services
- Exercise, nutrition, and weight management
- Psychometric assessments and trauma debriefing
- Managerial coaching and team enrichment sessions
Disease and Disability Management:
- Chronic disease management for conditions like diabetes and hypertension
- Coordinated healthcare interventions and communications
- Disability Management Programme for workplace reintegration
Employee Wellness Training (Voluntary):
- HIV/AIDS training
- Financial wellness training
- Conflict management and motivational training
- Stress management and depression training
- Alcohol and drug abuse control training
Primary Health Care (On-Site):
- 2 hours per week (8 hours per month)
- Management of IOD cases
- Coordination of quarterly wellness days (4 per annum)
- Employee referrals and minor ailment treatment
- Must be able to dispense stat medication
Occupational Health:
- Audiometric testing
- Vision/eye testing
- Lung testing
- Annual medical screening for specific positions

Mandatory Documents
Failure to submit these documents will lead to bids being deemed non-responsive:
- Company registration (CK) documents and certified ID copies of directors/owners (certification not older than 6 months)
- Fully completed and accurate MBD forms 4, 6.1, 8 and 9
- Bidder’s municipal statement of account covering 3 months or valid lease agreement
- Directors’ municipal statements of account covering 3 months or valid lease agreement
- If unavailable, an affidavit declaring no municipal arrears for more than 90 days
- Proof of registration on Central Supplier Database (CSD)
Additional Requirements:
- B-BBEE Status Level Verification Certificate or Sworn Affidavit (original or certified copy)
- Proof of address of office dealing with the project (lease agreement/municipal account)
- For bids above R10 million: Financial statements for past 3 years
- All pages must be originally signed in black ink
Evaluation Criteria
The bid will be evaluated in two stages:
Stage 1: Functionality Assessment (Minimum 70% threshold)
- Relevant Company Experience: 40 points (10 or more projects: 40; 5-9 projects: 35; less than 5: 20)
- Approach/Methodology: 20 points
- Individual/Team Capability: 40 points (10 or more years experience: 40; 5-9 years: 35; less than 5: 20)
Stage 2: 80/20 Preference Point System
- Price: 80 Points
- B-BBEE Status Level of Contribution: 10 Points (Level 1: 10; Level 2: 9; Level 3: 8; Level 4: 6; Level 5: 4; Level 6: 3; Level 7: 2; Level 8: 1; Non-compliant: 0)
- Locality: 10 Points (Within SBDM jurisdiction: 10; Within Eastern Cape: 8; Within South Africa: 5; Outside South Africa: 1)
Competency Requirements
Professional Registration:
All health professionals must be registered with recognised professional bodies:
- Health Professions Council of South Africa (HPCSA)
- South African Nursing Council (SANC)
- South African Council for Social Service Professions (SACSSP)
Required Team Members:
- Medical Doctor
- Nurse
- Social Worker
- Occupational Health Medical Doctor
- Counselling Psychologist
Proof Required:
- Detailed CVs with certified attachments
- Evidence of valid professional registration
- Detailed organogram linked to personnel and project responsibilities
