Scope of Work
The appointed service provider will conduct comprehensive medical assessments to support employee health, safety, and legal compliance. Services include:
- Entry and exit medical examinations
- Annual eye tests, lung function tests (twice per year), and audiometric tests
- Tetanus injections and drug/alcohol dependency documentation
- Medical evaluations for incapacity investigations and disability applications
- Full medical examinations including hearing, lung, vision, blood pressure, glucose, urine, chest X-rays, and ECG
- First aid training, incident investigations, hazard identification, and OHS induction
Mandatory Documents
Bidders must submit the following compulsory documents in Section A of the checklist:
- Signed Form of Offer and Acceptance
- Completed Pricing Schedule and MBD 1, 4, 6.1, 8, and 9
- Schedule 1A (Authority of Signatory) and 1B (Enterprise Questionnaire)
- Proof of HPCSA registration and CSD (MAAA number)
- Valid SARS TCS PIN and Municipal Rates & Taxes account (not older than 3 months)
Evaluation and Compliance
The tender uses the 80/20 preference point system. Bidders must first achieve a minimum functionality score of 75 out of 100 points to be considered responsive. Functionality is scored on:
- Experience and Capacity (30 points): Years in operation, completed projects, company profile
- Qualifications of Key Staff (30 points): Certified qualifications, HPCSA registration, CVs
- Methodology and Quality Assurance (20 points): Service delivery plan, quality measures
- Presentation (20 points): Proposed approach and added value
Specific Goals and Preference Points
Preference points are allocated for:
- Local Economic Development (10 points): Registered office within Phokwane (10 pts), Frances Baard District (7 pts), Northern Cape (4 pts), outside (2 pts)
- Women Ownership (5 points): 100% (5 pts), 31-50% (3 pts)
- Youth Ownership (5 points): 100% (5 pts), 31-50% (3 pts)
