  • Latest municipal account or lease agreement
  • CIPRO registration documents (with member percentages for CCs)
  • Share certificates and shareholding breakdown (race, gender, percentage)
  • Declaration of Interest (SBD 4)
  • SANAS-accredited B-BBEE certificate or sworn affidavit (EMEs/QSEs)
  • Proof of retail shop or manufacturer’s declaration for PPE
  • Completed price schedule with detailed breakdown
